I am going abroad for Spring Break, but I can't find the foreign travel form on the new website. Does anyone know where it lives?

I would send an email to smart, they'll tell you if it's necessary. I believe that you only have to inform the government and smart of foreign travel if you have a security clearance or if you're studying abroad. But I'd ask to be sure.

I sent an email to SMART and then you email it back to your POC
