Site Visit Report Form

Hey,
So I just finished my site visit but I can't find the download link on the portal for the site visit report form. I also was not given anything while I was there to fill out that would be report related. Can anyone point me in the right direction?

Try emailing your LMI liaison, that’s how I received my form

How do we know who our LMI Liason is?

If you don't know who your liaison is, then I think the best option would be to email LMI at smart@lmi.org and ask for the form directly. Hope this helps!

Hey!
So I found the form on the portal, it's hidden in the handbook. It's a separate file in the page.

From portal view go to Progam Award Docs > Scholar Handbook.

This is where I found it. Of course I go back now and it's no longer there. Maybe it was just a fluke. I would try looking through the program award docs and see if it is attached to another task.

The Site Visit form is emailed directly to you from the SC at your SF. It is not available in your Documents on the portal. If you haven't received it, request it.
