SMART Acceptance FORMS

Is there anywhere that explains how to correctly fill out the forms? Section 12 of the service agreement form says "Component Administrative Officer SMART" and requires a signature. Do we leave that blank? Also for Form 612, what is our "Job Title in Announcement"? Where can I go to find the resources to know that I am properly filling out these forms? Thanks!

for forms that require a signature, you MUST in my experience print the document and then SCAN it, then submit the scanned form.

I already asked SMART about this today.

Job Title: "SMART Scholarship"
Grades Applying for: NA
Announcement Number: NA

I think this forum is just for HR so they know to put us on payroll

I received a response for that from the SMART help desk and they said to leave section 12 of the service agreement blank. The SMART office signs that section after you submit your forma.

Should I complete the forms by hand or type them? do they prefer that you do it a specific way ?

I put applicant but not 100% certain.

For form 612, if I don't have any related work experience (only worked at fast food places in the past), should I just leave the work experience sections blank?
