I had this same issue, and made several trips to the grad studies office to get it squared away. I'd encourage you to find out how the sponsor letter/funding process works at your university, know the people by name and follow up with them each year.

For my university (UC) it works like this:
There is a grad fellowship analyst who I submit my sponsor letter to, then she submits the letter to the billing department who will "rebudget" the account showing that my tuition is paid, and then an invoice for the tuition is sent to the SMART office.

This quarter, by mistake the fellowship analyst had not sent over the sponsor letter to billing.

My school posted it so I didn't have any holds on my account at the beginning of the semester but they still haven't billed SMART since add drop just ended.

I got mine paid last week. Just FYI though, if your a grad student, and your school didn't bill it right, there will be a balance for the stipend because school is unclear who pays that. I just told my department that SMART pays me directly, and they waived the GSR (grad student researcher stipend).
